As we’re in the midst of a global pandemic, we cannot deny the fact of the widespread use of anti-inflammatory agents in treating symptoms. The early study of using NSAIDS showed that it may even exacerbated the symptoms, particularly with the ibuprofen. The debate is still going on. The mechanism could be through upregulating the angiotensin converting enzyme 2 (ACE2) receptors. And there is also a possible theory that NSAIDS might delay the diagnosis of Covid-19 by masking inflammation and fever. After several studies, WHO recommended not to use the ibuprofen but it’s permissible only with careful monitoring of the risk.
After doing several research, the use of NSAIDS is not associated either with the mortality, critical care admission, comorbidity or positive outcomes of the disease. The study was made by evaluating the assessment 2 weeks before the admission. There wasn’t enough to prove the benefits or adverse effects of using NSAIDS during the hospital stay.
Nevertheless, the mild NSAIDS like paracetamol will still be used largely as symptomatic relief. Though one must be careful about the concomitant usage with other therapeutic drugs, in the danger of drug interaction and stomach side effects.
To be concluded, several previous observational studies indicated that there is a lack of association between NSAID use and Covid severity.
Based on my personal opinion, clinicians should not refrain from the use of NSAID in Covid unless there are adverse reactions.
